What is the file format of a PMAP?

A Prescription Map (or PMAP) requires a pmh (Prescription Map Header) and a pmd (Prescription Map Data) file to operate properly.  Bothe files must have the same name.  PMAPs can be created in MapStar from SHAPE files.
